Problem with the page breaks inside a table

Hello Experts,

I want to print only 3 line items per page in  a smart form, Please note I am using Table to print the line items and not using loop.

The requirement is there are 5 blank boxes  after each line item. I am not able to insert program lines inside a table.

I am able to insert either before the start of table
